How to Submit Samples

Before sending samples to Selvita, please follow these steps:

1. Download the sample submission form.
2. Fill out the form clearly.
3. Send the completed form by e-mail to your designated contact person at Selvita.

Additionally, please include a printed copy of the sample submission form within the package along with the samples. For detailed instructions on how to complete the form, please refer to page 3 of the document.

Please include the SDS (Safety Data Sheet) for the samples within the package.
